When insurance is used, ZIKS Health bills your insurance company for covered services. Patients are responsible for any copay, deductible, or non-covered balance determined by their plan.
Payment Expectations
Estimated patient responsibility is generally due before or at the time of service. ZIKS Health accepts major debit and credit cards, and a valid payment card must remain on file.
Self-Pay & Good Faith Estimate
Transparent self-pay rates are available for patients who do not have insurance or choose not to use insurance. You have the right to receive a Good Faith Estimate of expected charges prior to care.
